New Student Enrollment Form

New Students:

Once you have had your trial lesson, and are ready to enroll, use this form to setup your account.

After you have submitted this enrollment form, we will finalize your account, and activate your login.

The enrollment deposit is $200. If you are a guitar student enrolling for the first time, All or part of $100 of this enrollment deposit is used to cover the cost of your equipment (not including the guitar) with the balance showing as a credit on your first registration. For non guitar students, $100 is applied as a credit on your first registration.

The balance of $100 is held as a security deposit, as we provide all students with the necessary books & materials throughout the year, and these items are invoiced once per year at the end of the spring semester.
